Within the epineurium there is a connective tissue matrix called the internal epineurium and fascicles. The internal epineurium acts as soft cushion for the fascicles. A nerve fiber may have a variable number of fascicles, but there will be at least one (otherwise there would be no nerve cells). Fascicles are surrounded by a tissue layer called the perineurium which is a protective sheath that acts as a barrier. Inside the fascicles is the endoneurium, a tissue matrix analogous to the internal epineurium, and the nerve cells. The endoneurium has many small blood capillaries (endoneurial microvessels) which directly supply the nerves themselves. These capillaries have tight junctions to prevent the free flow of materials between cells and instead require substances to pass through the endothelial cells.

=== Lymphangioleiomyomatosis === In May 2015, the FDA approved sirolimus to treat lymphangioleiomyomatosis (LAM), a rare, progressive lung disease that primarily affects women of childbearing age. This made sirolimus the first drug approved to treat this disease. LAM involves lung tissue infiltration with smooth muscle-like cells with mutations of the tuberous sclerosis complex gene (TSC2). Loss of TSC2 gene function activates the mTOR signaling pathway, resulting in the release of lymphangiogenic growth factors. Sirolimus blocks this pathway. The safety and efficacy of sirolimus treatment of LAM were investigated in clinical trials that compared sirolimus treatment with a placebo group in 89 patients for 12 months. The patients were observed for 12 months after the treatment had ended. The most commonly reported side effects of sirolimus treatment of LAM were mouth and lip ulcers, diarrhea, abdominal pain, nausea, sore throat, acne, chest pain, leg swelling, upper respiratory tract infection, headache, dizziness, muscle pain and elevated cholesterol. Serious side effects including hypersensitivity and swelling (edema) have been observed in renal transplant patients. While sirolimus was considered for treatment of LAM, it received orphan drug designation status because LAM is a rare condition. The safety of LAM treatment by sirolimus in people younger than 18 years old has not been tested.
== Mental health assistance == Due to the discrimination they face, trans individuals often present significant mental health disparities. Among them, it is estimated that 32-50% of trans individuals across various countries have attempted suicide, with this high prevalence attributed to factors such as victimization, bullying, violence, social and familial rejection, as well as discrimination in different public sectors. Other than gender dysphoria, trans individuals are also affected by mental health diagnoses, such as major depression and generalized anxiety, at far greater rates than the non-trans population. Whilst gender-affirming care has a positive impact on trans individuals' mental health and may lessen several symptoms, the psychological improvements of such care are often limited by the minority stressors affecting the trans population. Due to these factors, mental health assistance through means such as psychotherapy may play an important role in addressing trans patients' mental health. Mental health assessments and treatment may be conducted prior to the initiation of gender-affirming care as a means of ensuring the patient's informed consent and complementing medical transition. Pyschotherapy as a requirement for the inititation of affirmative medical care today is discouraged by trans health care protocols, as are approaches attempting to change the patient's identity or skew self-exploration towards one particular direction.
== Signs and symptoms == The first symptom of CJD is usually rapidly progressive dementia, leading to memory loss, personality changes, and hallucinations. Myoclonus (jerky movements) typically occurs in 90% of cases, but may be absent at initial onset. Other frequently occurring features include anxiety, depression, paranoia, obsessive-compulsive symptoms, and psychosis. This is accompanied by physical problems such as speech impairment, balance and coordination dysfunction (ataxia), changes in gait, and rigid posture. In most people with CJD, these symptoms are accompanied by involuntary movements. Rarely, unusual symptoms like the alien limb phenomenon have been observed. The duration of the disease varies greatly, but sporadic (non-inherited) CJD can be fatal within months or even weeks. Most affected people die six months after initial symptoms appear, often of pneumonia due to impaired coughing reflexes. About 15% of people with CJD survive for two or more years. The symptoms of CJD are caused by the progressive death of the brain's nerve cells, which are associated with the build-up of abnormal prion proteins forming in the brain. When brain tissue from a person with CJD is examined under a microscope, many tiny holes can be seen where the nerve cells have died. Parts of the brain may resemble a sponge where the prions were infecting the areas of the brain.
